The 2012 Business Moneyfacts Awards - Survey of UK Businesses
The questions below are based on your relationship with providers and brokers between December 2010 and the present. Please spare a few minutes to complete this online survey and put yourself in the running to win a personal wine consultation and £150 to spend with our wine merchant - 'The Perfect Wine'.
For your responses to be valid and to be entered into the prize draw, please complete the survey by Friday 11 November 2011. The first name drawn at random will win the wine consultation and voucher. You might be our lucky winner!
The Ceremony

Survey Notes
- For each category, businesses should give one name in each area that they believe has offered the best service.
- The first three questions relate to financial service providers, the latter questions to business and commercial finance brokers/advisers.
- You need to consider whether your provider or broker has lived up to the promised levels of service offered, if they have answered your calls for help and supported you over the last year. Would you recommend each provider to fellow businesses?
- It is not essential to list a provider in every category.
- Ensure that you press 'Finish' when complete.

Prize Draw Terms and Conditions
Please read these prize draw rules carefully. If you enter our prize draw, we will assume that you have read these rules and that you agree to them.
To enter the prize draw you must represent a business registered in the UK.
The prize draw is not open to employees (or members of their immediate families) of Moneyfacts Group.
There is no charge for entering.
Only one entry per person.
The closing date is midnight on Friday 11 November 2011, and entries received after this point will not be eligible for the prize draw.
The prize is a personalised consultation with The Perfect Wine and then your choice from their selection of wines to the value of £150.
The winner will be selected at random from all eligible entries received. The draw will take place on Tuesday 15 November 2011. The winners will be notified on or shortly after this date by email. If the prize is unclaimed or declined within a reasonable period, a supplementary winner may be drawn at Moneyfacts’ discretion and notified by email. Moneyfacts will not be responsible for any inability of the prize winner to take up the specified prize.
You can find out who has won the prize draw by sending a stamped addressed envelope marked ILP Prize Draw to: Moneyfacts Group plc, Moneyfacts House, 66 – 70 Thorpe Road, Norwich NR1 1BJ.
Moneyfacts reserves the right to provide a substitute prize of similar value, should the specified prize become unavailable for reasons beyond its control.
Cash or credit alternatives will not be offered.
The prize is not transferable.
By entering the prize draw the winner agrees to participate in such promotional activity and material as Moneyfacts Group plc may require.
Entrants must have satisfactorily completed the Awards Questionnaire to be eligible for the prize draw.
This competition is being run by Moneyfacts Group plc, Moneyfacts House, 66 – 70 Thorpe Road, Norwich NR1 1BJ.
Moneyfacts’ decision is final and no correspondence will be entered into.
Moneyfacts Group reserves the right to amend these rules at any time.
Moneyfacts Group will endeavour to send prizes or prize vouchers within a month of the prize draw closing date but cannot guarantee this delivery time.
